Hauler Registration Form

The Logan Township Municipal Utilities Authority must have the following documents attached to this completed registration form. It is also the company’s responsibility to make sure the Authority has up to date NJDEP registrations and insurance documentation. This form needs to be filled out for each truck entering the Authority. If you have any questions, please call (856) 467-1650, ext. 112 or ext. 116.

• NJDEP Registration Card for Business
• Certificate of Liability Insurance Policy
• Automobile Insurance Card (if not included on Cert. of Liability)
• If planning on bringing half loads, we need a tank chart for each tanker from the manufacturer.
